The first step is an assessment session. I will do my best to give you a diagnosis and specific recommendations.
The purpose of the initial evaluation is to determine what kind of treatment is indicated for your condition. It is possible that further treatment with me may not be indicated.
Please go onto my website, click on the “forms” tab, and print out all the forms in the “everyone” section and in the “adult” section. Fill out ALL of the forms. Please bring the completed forms to our first meeting.

9 Cedar St. is an old yellow house on the far left-hand corner of the intersection of Cedar and Linden. There is free parking next to the building on the Linden St. side. I am on the first floor.

Take Rt. 9/ Belmont St. exit in Worcester
Take Rt. 9 West
You will soon go through intersection at Lincoln Square: you will go through two sets of lights in Lincoln Square. You will then will start up a hill and go through two more sets of lights before coming to a third set of lights at Lancaster St.
Take a left on Lancaster St.
At your first set of lights, Lancaster St. will intersect with Cedar St. At this intersection, 9 Cedar St. will be a yellow Victorian house diagonally across to your left.
There are several parking options for 9 Cedar St:
1) At the set of lights (above), turn left and there are metered spaces in front of the building.
2) At the set of lights (above) turn right and there will be two parking lots immediately on your left. Take the second entryway and park to your right, facing the fence at the end of the lot.
3) At the set of lights (above), go straight and you will see parking spaces along the side of the building. When facing the building, WCCBT spaces are the first two spaces on the left.
Please do not park in spaces or parking lots other than indicated above to avoid the risk of being towed.
